Thank you for being part of HCHS/SOL. You are one of 16,000 participants nationally representing the Hispanics/Latinos of Chicago.
Your participation in this part of the study is called follow-up phase in which you will be called or visited at home to ask you questions about your health status, lifestyles practices and nutrition. This call or visit only will take a few minutes of your time, but the information we collect will be of great value for the study. Also you will be asked about your visits to Emergency Room and/or if you have been hospitalized for some medical reason. Because all this is important information for the study, we suggest keeping a record of your hospital visits particularly Emergency Room visits and hospital overnight stays. Thanks to you the study continues and you are helping us to learn more about the health of Hispanics/Latinos living in the U.S. Please let us know changes in your home address or telephone number.
